Thank you for being an affiliate of Commercial Solutions and welcome to the Affiliate Portal! This online hub is where you will be able to find any forms you need for our Compliance Department. All affiliate compliance forms can be sent to compliance@commercialsolutions.com and any workorder documents like pictures, signoffs and invoices can be sent to documents@commercialsolutions.com.
If you’d like to fax any documents over please fax to (401)-353-8070 and please include any information as to what the document is, who it is for and if it associates with any specific workorder(s).
Required Compliance Documents Prior to Receiving a Commercial Solutions Workorder:
- ⬢ Up-To-Date Certificate of Insurance
- ⬢ W-9 Tax ID Form

- ⬡ If your company has multiple physical locations please also complete our Multiple Locations Affiliate Agreement Form.
